Testosterone production is the result of a sequence of well-coordinated events. Gonadotropin releasing hormone (GnRH) is secreted in a pulsatile fashion from the hypothalamus. It is transported to the pituitary and binds to the surface of pituitary gonadotropes triggering a series of intracellular events that result in the secretion of both luteinizing hormone (LH) and follicle-stimulating hormone (FSH). LH released from the pituitary has a half-life of 60 to 90 minutes. It binds to its cognate receptor on the surface of a Leydig cell to initiate steroidogenesis.

Testosterone replacement is the suggested therapy for men with biochemically diagnosed TDS.
The goal of treatment is to improve symptoms through achievement of physiological levels of testosterone. Several safe and effective formulations are available and are listed in terms of their invasiveness. Testosterone products exist as injections (testosterone enanthate or testosterone cypionate), pills (testosterone undecanoate) patches, gels and topical solutions. The choice of testosterone product should be discussed with the patient and based upon the ASTEP algorithm: Availability, Safety, Tolerability, Efficacy and Preference.
